Acknowledgement of Risk and Release of Responsibility

One of the University’s four core missions is Public Engagement. When units invite the public to participate in special programs, camps, and various other activities it may expose the University to additional risk. Likewise, there may be times when student programming creates the potential for additional and/or unusual risks; a field trip for example. It is important that participants are made aware of the potential risks of an activity so that they may make informed decisions about taking part in events.

Risk Management and Legal Counsel have collaborated on several Acknowledgement of Risk and Release of Responsibility forms that units can use for their special programs. The unit may customize the form with details of their event and include hazards that may be associated with the activity. In addition, the form contains a waiver of responsibility in favor of the University.
