Acknowledgement of Risk and Release of Responsibility

One of the system’s four core missions is Public Engagement. When units invite the public to participate in special programs, camps, and other activities it may expose the system to additional risk. Likewise, student programming may create the potential for additional or unusual risk, such as on a field trip. It is important that participants are made aware of the potential risks of an activity so that they may make informed decisions about taking part in events.

Risk Management and University Legal Counsel have collaborated on several Acknowledgement of Risk and Release of Responsibility forms that units can use for their special programs. The unit may customize the form with details of its event and include hazards that may be associated with the activity. In addition, the form contains a waiver of responsibility in favor of the system.
